There is genuine human controlled data, but not for longevity. The strongest available study (Bahenský et al. context aside, the key trial is the German long COVID study, PMC11634465) was a prospective, controlled, OPEN-TREATMENT, NON-RANDOMISED pilot in 145 inpatients at a single German rehabilitation centre. The IHHT arm (n=70) improved 6-minute walk distance 2.8-fold more than standard care (91.7 m vs 32.6 m, p<0.001), with secondary gains in stair-climbing power, dyspnoea, fatigue, quality of life and handgrip. The authors themselves state the design had limited strength due to absence of blinding and non-random assignment, producing baseline imbalance between groups. So: this is deconditioned post-viral patients regaining function during a five-week inpatient rehab programme, not healthy adults gaining healthspan. What the evidence does NOT show: any effect on mortality, biological age, epigenetic clocks, or any longevity or healthspan endpoint; any benefit in healthy or asymptomatic people; any durability beyond the treatment window. The mitochondrial-rejuvenation and anti-ageing claims made in clinic marketing are mechanistic extrapolation, not demonstrated outcomes. Safety signals appear benign in supervised use, though the "no reported adverse events" claim cited by at least one Australian provider rests on an unpublished 2015 literature review, which is not verifiable evidence. Note also that provision in Australia sits mainly with naturopathic and wellness practices rather than medical longevity clinics, which is itself informative about how the intervention is positioned.

What it is
IHHT is a breathing protocol in which a person alternates, through a mask, between inhaling low-oxygen air (roughly 10 to 12 percent O2, simulating altitude) and oxygen-enriched air (roughly 30 to 35 percent O2), in cycles over a 30 to 60 minute passive session. In Australia it is delivered almost entirely via the ReOxy device, developed in Luxembourg and manufactured in Germany, which adjusts the hypoxic dose to the individual's measured response. Providers claim it improves mitochondrial function, cellular energy metabolism, endurance, stress resilience, sleep, cognition and fat metabolism; the longevity framing is built on the mitochondrial and hormesis story rather than on longevity outcome data.
